Audience: Professionals who determine functional eligibility for children in the state of Wisconsin for long term support programs who have experience regarding the unique conditions, development, needs, and functioning of children with significant disabilities.


Registration Instructions: Registration for this program is only by access code; given to professionals working with children in Wisconsin's long-term support programs.


Purpose: This program focuses on providing the required instruction to become a certified Children's Long Term Support functional screener in Wisconsin.

Objectives: Upon completion, the certified screener will be able to:
  1. Have an overview of the CLTS Functional Screen history and purpose;
  2. Accurately provide the demographic and identification information on the Informational pages;
  3. Know how to use the Diagnoses Cue Sheet and list current diagnoses and identify presenting diagnoses;
  4. Complete the Mental Health page with consideration to duration, symptoms, services and rare circumstances;
  5. Identify and record the frequency of specific behaviors and the current interventions being used;
  6. Capture every child's significant functional impairments in Activities of Daily Living and Instrumental Activities of Daily Living;
  7. Answer the required questions on the work and school page of the functional screen;
  8. Apply their knowledge of health-related services;
  9. Assess risk involved in children's family's homes;
  10. Understand the Functional Disability Indicator;
  11. Provide the necessary screen completion information; and
  12. Properly interpret and use the final functional eligibility results.
How successful completion of program is accomplished:
Successful completion is accomplished by earning a minimum score of 80% for each of the tests following individual course modules. After completing the evaluation, your certificate of completion may be viewed and printed for record-keeping. Directions are then provided to gain access to the CLTS Functional Screen.

6-Part II Instrumental Activities of Daily Living (IADLs) 1.5 hours
 7 Work and School .5 hours
 8 Health-Related Services 1.5 hours
 9-11 Risk, Functional Eligibility Indicator (FDI), and Screen Completion Information .75 hours
12 Functional Eligibility Results .5 hours


Computer Requirements:
  • Windows Vista, XP or 2000
  • Internet Explorer 5.5 or above
  • Adobe Acrobat Reader 5 or above
  • Windows Media Player 9 or above
  • Appropriate equipment and software to view a webcast. (If you have never seen a webcast, please ensure that your computer meets all of the prerequisites before beginning this course.)
